The following is a list of Publications the Council has developed to assist home care agencies understand and comply with the federal guidelines and industry standards regarding the provision of home care aide services in Geneva.

    TRANSITIONAL CARE IN-SERVICE TRAINING CURRICULUM  
    --Developed by the Standards Committee of the MA Council for Home Care Aide Services.
    --Designed for use by an educator and/or supervisor at a home care agency to provide a three-hour training to home care aides on Transitional Care and Chronic Disease Management.
    --Includes a companion resource disk with electronic copies of all materials. 
    --Curriculum is divided into four areas: 1. Facilitator’s Guide that includes Learning Objectives, Content Summary, Activities, and Resources; 2. PowerPoint Presentation; 3. Transitional Care Handouts; “The Big 10: Keys to Keeping Consumers at Home” Chronic Disease Management Fact Sheets each of which can be used as a tool to create additional in-service training for each disease/condition.
